Accident Summary:

 AT APPROXIMATELY 1600 HOURS ON 13 AUGUST 1971, AIRCRAFT ^67-17834^, A UH-1H, DEPARTED THE ^176TH AHC^ ON THE THIRD CHECKRIDE OF THE DAY. IT HAD BEEN AN UNEVENTFUL DAY UP TO THIS POINT. NORMAL, STEEP APPROACHES, HYDRAULICS OFF LANDINGS, AND A DEMONSTRATION STUCK PEDAL SETTING HAS ALREADY BEEN ACCOMPLISHED UP TO THIS POINT. APPROXIMATELY 50 MINUTES INTO THE THIRD CHECKRIDE, WHILE THE PILOT WAS AT THE CONTROLS, THERE WAS A LOUD GRINDING NOISE HEARD IN THE AFT SECTION OF THE AIRCRAFT ACCOMPANIED BY A VIBRATION OF THE WHOLE AIRCRAFT. THE INSTRUCTOR PILOT IMMEDIATELY TOOK CONTROL OF THE AIRCRAFT AND CALLED CHU LAI EAST TOWER TO INFORM THE TOWER HE WAS HAVING DIFFICULTY WITH THE AIRCRAFT. THE IP THEN TURNED TO THE LEFT AND LOWERED PITCH TO MAKE A LANDING ON RUNWAY 210 AT CHU LAI EAST. THE AIRSPEED OF THE AIRCRAFT WAS ABOUT 60 KNOTS AT THIS TIME AND THE AIRCRAFT WAS SLOWING DESCENDING IN A LEFT HAND TURN. WHEN THE IP LOWERED THE COLLECTIVE THE GRINDING NOISE WAS HEARD AND THE HELICOPTER VIBRATED AGAIN. THE AIRCRAFT SUDDENLY PITCHED UP AND TURNED SEVERELY TO THE RIGHT. THE IP APPLIED RIGHT AND FORWARD CYCLIC. THIS APPLICATION OF CYCLIC BROUGHT THE NOSE OF THE AIRCRAFT DOWN AND INDUCED AN UNCONTROLLALBE SPIN TO THE RIGHT. THE IP TRIED TO GAIN CONTROL OF THE AIRCRAFT, BUT THE AIRCRAFT WOULD NOT RESPOND TO ANY CYCLIC CONTROL MOVEMENTS. AT APPROXIMATELY 100 FEET AGL, THE IP PULLED PITCH AND THIS BROUGHT THE AIRCRAFT TO A LEVEL ATTITUDE, BUT IT WAS STILL SPINNING TO THE RIGHT. THE IP TRIED TO INCREASE THE RADIUS OF THE TURN AND BUILD UP ENOUGH AIRSPEED TO FLY IT OUT, BUT HE DID NOT HAVE ENOUGH ALTITUDE. JUST BEFORE IMPACT WITH THE GROUND, THE IP PULLED PITCH TO CUSHION THE LANDING AND UPON CONTACT WITH THE GROUND, THE AIRCRAFT ROLLED ON ITS SIDE, SHEARING THE MAST, CAUSING MAJOR DAMAGE. THERE WAS NO FIRE OR FUEL LEAKAGE. THERE WERE NO INJURIES TO THE CREW AND THE CREW WAS WEARING NOMEX FLIGHT CLOTHING AND OTHER REQUIRED PROTECTIVE GEAR.\\
