Accident Summary:
ON 20 MARCH 1970 AT 1230 HOURS, WO STONE DEPARTED THE 176TH ASSAULT HELICOPTER COMPANY ON THE FIRST OF THREE (3) SCHEDULED STANDARDIZATION CHECK RIDES. THE FIRST TWO CHECK RIDES WERE EACH FLOWN FOR A PERIOD OF 1:30 AND A TOTAL OF SIX PRACTICE STRAIGHT IN AUTOROTATIONS WERE COMPLETED PRIOR TO THE BEGINNING OF A 90 DAY STANDARDIZATION CHECKRIDE WITH WO EDWARDS. THAT FLIGHT BEGAN ABOUT 1600 HOURS WITH A DEPARTURE FROM THE 176TH RAMP AND A TWO (2) MILE FLIGHT TO CHU LAI EAST AIRFIELD. THE AIRCRAFT ENTERED TRAFFIC FOR RUNWAY 32 AND THE INSTRUCTOR PILOT DEMONSTRATED THE FIRST AUTOROTATION EXPLAINING THE STANDARD TECHNIQUE REQUIRED AS THE MANEUVER WAS BEING PERFORMED. AFTER COMPLETION OF THE AUTOROTATION AND A DISCUSSION OF THE APPROPRIATE TECHNIQUE, WO STONE THEN DEMONSTRATED A LOW LEVEL AUTOROTATION AND A SIMILIAR DISCUSSION FOLLOWED. WO EDWARDS MADE THE NEXT TAKE OFF AND TRAFFIC PATTERN. ON HIS FINAL LEG HE ALIGNED WITH RUNWAY 32 AND MADE AN APPROPRIATE ENTRY FOR A STRAIGHT-IN AUTOROTATION. THE AUTOROTATION APPEARED NORMAL UNTIL AN ALTITUDE OF ABOUT 10 TO 15 FEET WHEN IT WAS TIME FOR THE INITIAL COLLECTIVE PITCH APPLICATION AND THE NECESSARY LEVELING OF THE AIRCRAFT JUST PRIOR TO TOUCHDOWN. DURING THIS PORTION OF THE MANEUVER THE PILOT WAITED TOO LONG TO LEVEL THE AIRCRAFT AND THE TAIL ROTOR STRUCK THE RUNWAY. ONE TAIL ROTOR BLADE BROKE AWAY FROM THE HUB AND THE AIRCRAFT WENT INTO A 360 DEGREE YAW TO THE RIGHT AS A TOUCHDOWN WAS COMPLETED. A NORMAL SHUTDOWN WAS PERFORMED AS THERE WERE NO INJURIES OR INDICATION OF FIRE.\\
